This form is intended for use by school counsellors and administrators who are unable to submit e-transcripts to the University of Toronto through an electronic transcript system (e.g. the Ontario Universities’ Application Centre, Parchment/Naviance, Scoir, MyCreds, etc.). Applicants to U of T should not use this form to submit documents.
The University of Toronto routinely verifies the authenticity of information submitted in support of an application for admission, and reserves the right to determine whether documents submitted satisfy application requirements.
This form is for one student only; if you have multiple student transcripts to upload, you must submit this form multiple times.
Please upload only transcripts or other academic documents. Do not upload non-academic documents (e.g. letters of reference, resumes).
Uploaded transcripts must include the student's full name and date of birth.
